Care Coordinator:

The Winfield at Middletown is seeking a Care Coordinator. This person will be responsible for maintaining an efficient, accurate schedule and updating the schedule to reflect shift trades, terminations, and leaves of absence. This person iwll track call-offs, late arrvals, and early departures as they occur. Meet and provide staffing reports to the HWD so the team can guarantee the Time and Attendance Policy is followed.

Qualifications:

• Ability to work a flexible schedule to allow for shift coverage when needed

• great communication and team building skills

• ability to work with directors within the parameters of corporate policies and procedures

• ability to lift 35 pounds regulary and up to 50 pounds intermittently

• must be detailed oriented

• flexible to frequent schedule changes

• basic computer knowledge

• independent thinker who can problem solve

Experience:

• High School Diploma / GED

• 2 years of experience in long term care, hospice care, or healthcare setting required
