Location Requirement:

Brevard Countym FL (Palm Bay or Melbourne areas)

Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Pay Rate: $34.13/hour + mileage reimbursement
Type: 6-Month Contract-to-Hire

Job Summary:
We are seeking a compassionate and organized Bilingual (English & Spanish) Care Manager to provide face-to-face case management services to members in the Long-Term Services and Support (LTSS) program. This role requires traveling throughout Brevard County (75% field-based) to conduct in-home assessments, coordinate care plans, and collaborate with healthcare providers to ensure members receive the right care at the right time.

Key Responsibilities:

Conduct in-home visits using standardized assessment tools
Develop and maintain member care plans in collaboration with physicians and support teams
Authorize services within LTSS guidelines in a cost-effective manner
Schedule and attend interdisciplinary team meetings
Advocate for members and ensure safe transitions of care
Maintain accurate, timely documentation in electronic health records
Apply critical thinking and problem-solving to support member needs

Qualifications:

Bachelor’s degree in psychology, social work, counseling, sociology, human development, or early childhood education
OR non-licensed Master’s level clinician in a related field
At least 2 years of field case management experience preferred
Bilingual (Spanish) required
Must have reliable transportation and willingness to travel locally
