Reports To: Clinical Director / Administrator & Intake SupervisorPosition Summary:The Intake Coordinator / Medical Assistant plays a critical dual role in managing new patient admissions and providing clinical and administrative support to the home health and hospice teams. This position ensures that all referrals are processed efficiently and accurately while upholding high standards of patient care and compliance. Medical assistant experience is essential to perform clinical tasks and facilitate smooth transitions for patients into our services.Key Responsibilities:Intake Coordination:
- Receive and process referrals for home health and hospice services.
- Verify insurance eligibility and obtain prior authorizations as required.
- Coordinate with physicians, hospitals, discharge planners, and other referral sources.
- Gather and review all necessary documentation for admission (e.g., face-to-face encounters, orders, H&P).
- Enter patient information into EMR system accurately and in a timely manner.
- Schedule admissions and communicate with clinical teams for assignment.
- Maintain HIPAA compliance and ensure all documentation meets regulatory requirements.
- Support clinical team with vital sign monitoring, chart preparation, and communication of medical history, medical orders preparation.
- Contact patients and families to gather relevant health information and address questions about services.
- Provide administrative support for clinicians, including lab follow-up, DME orders, and pharmacy coordination.
- Assist with patient education under supervision of licensed clinicians.
- Maintain medical supplies and ensure proper equipment is available for initial visits.
- Answer multi-line phone system and direct calls appropriately.
- Maintain organized and up-to-date patient intake logs and status updates.
- Support the billing team with documentation needed for claims processing.
- Collaborate with QA/QI team to ensure documentation accuracy for compliance audits.
- Medical Assistant Certification (CMA, RMA, or equivalent) – required.
- Minimum 2 years of medical assistant experience, preferably in a home health, hospice, or similar clinical setting.
- Minimum 1–2 years of experience as a Medical Assistant in a clinical, home health, or hospice setting
- Strong understanding of medical terminology and basic clinical protocols
- Familiarity with insurance verifications and prior authorization processes
- Experience with EMR systems (e.g., Kinnser, Axxess, HCHB) preferred
- Excellent communication skills, both verbal and written
- Ability to work independently and manage tasks under pressure
- Strong attention to detail and organizational skills
- Tech-savvy with the ability to operate software platforms, fax/email systems, and secure mobile/remote tools
- Compassionate and calm demeanor during sensitive patient or family interactions
- Primarily office-based.
