Location: Morton Plant

Status: Pool, Exempt: No

Shift Hours: 8:00am-4:30pm

Weekend Work: Every Other

On Call: No


Discharge Planner Pool - Responsibilities:

  • Provide assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
  • Possesses a clear understanding of discharge and transition planning and linkage with community resources.
  • Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
  • Provides support as active member of the treatment team.
  • Travel may be required.
  • Will perform other duties as assigned by supervisor.

Experience:

  • Required: 1 year of Behavioral Health and 2 years of Child and Adolescent (SIPP Only)
  • Preferred: 6 months in Acute Care

Education:
  • Required: Bachelor's in counseling
  • Or Bachelors in: Health Education, Social Work, Human Services, Psychology or Nursing
  • Preferred: Masters in human services

Credentials:
  • Required - Basic Life Support (Basic Life Support)
