SocialWorker-Hospice Care Warm Hearth at Home

Come be the spark at Warm Hearth! We're in search of an experiencedsocial worker to join our team atWarm Hearth at Home. The Social Worker serves as a key memberof the Interdisciplinary Team and is responsible for the providing support topeople who are terminally ill, their families and loved ones. Emotionalsupport is essential in order to improve the quality of life to those facingend-of-life. Candidates should possess asensitivity to ongoing cultural diversities and the needs of the aging population.Be trained in recognizing and assessing theemotional and social needs of those receiving care.

Keyresponsibility of this position includes but are not limited to the following:

Assesses the psychosocialstatus of patients and families/caregivers related to the patient's terminal illness and environment.

Educatesinterdisciplinary team on any special needsrelated to the culture of the patient andfamily.

Maintains clinical recordson all patients referred to social work prepares clinical and progress notes.

Provides information andreferral services for organization patients and families/caregivers regarding practical and environmental needs.

Serves as liaison betweenpatients and families/caregivers and community agencies.

Maintains collaborativerelationships with organization personnel to support patient care.

Participates in thedevelopment of the individualized plan of care, involving the patient and family

Actively participates inquality assessment performance improvement teams and activities.

Assists family and patientin planning for funeral arrangements, financial, legal, and health care decision responsibilities.

Other duties as delegatedby the DON.

This is a FT position, 40hours per week, Monday - Friday. Salary is competitive and dependent onexperience.

Requirements:

Masters of Social Work (MSW) degree

At least 1 year of Social Work experience in a healthcare setting.
