The Job:

We are currently recruiting for an entry-level Care Coordinator to join a dynamic team that serves underprivileged communities. The Care Coordinator will be responsible for providing direct support and coordinating care services for patients. The ideal candidate will have experience in direct patient care, or related education and possess strong organizational and communication skills with an empathetic mindset.

What You Will Be Doing As A Care Coordinator:
  • Coordinate patient care activities, including scheduling appointments, arranging referrals, and ensuring follow-up care
  • Maintain accurate and up-to-date patient records in electronic medical record systems
  • Communicate with patients, healthcare providers, and insurance companies to ensure seamless care coordination
  • Provide individuals with physical, emotional, and behavioral support allowing them to actively participate in activities offered while encouraging personal choices and assisting in maintaining their personal safety
  • Provide transportation, in personal and company-owned vehicles, throughout the day to and from the individuals' homes and activity locations in the community
  • Educate patients on available resources and support services

  • Customer service orientation-proactively assist clients in a professional, friendly manner to create the best possible experience by building relationships, understanding quality standards for service, and utilizing customer needs assessments
  • Social perceptiveness-be aware of other people's reactions and understand why they react the way they do. Maintain a positive, friendly composure even in difficult situations
  • Discretion-act with prudence and circumspection, especially in the handling of confidential and private documents/data.
  • Self-motivated-work autonomously to set and achieve targeted goals


Salary:
  • $38,000-42,000/yr + benefits


Requirements To Be A Care Coordinator:
  • Bachelor's Degree
