Substance Use Disorder Intake Coordinator
Department: Behavioral Health
Reports To: Behavioral Health Director
FLSA Status: Exempt
Location: Various Sites

Position Summary:
Family Health Center is seeking a passionate and organized Substance Use Disorder (SUD) Intake Coordinator to serve as a critical link between prospective patients, referral sources, and our clinic. This position ensures timely access to services, assesses eligibility for care, monitors ongoing engagement, and promotes our behavioral health offerings in the community. Key Responsibilities:

  • Serve as the primary intake contact for SUD services, completing initial ASAM assessments and scheduling follow-up evaluations.

  • Market and promote clinic services to community referral sources and prospective patients.

  • Facilitate access to services and track referrals and enrollments to ensure timely response and follow-up.

  • Monitor patient attendance at required services and conduct outreach to re-engage individuals as needed.

  • Develop and maintain tracking systems and databases for patient information and activity documentation.

  • Provide clear communication with clinic staff and community partners to support patient care coordination.

Qualifications:

  • Familiarity or ability to learn electronic health record (EHR) systems.

  • Knowledge of American Society of Addiction Medicine (ASAM) criteria is required.

  • Must have reliable transportation.

  • Effective verbal and written communication skills, especially with diverse populations and partner organizations.

Education & Experience:

  • Bachelor’s degree in human services (e.g., psychology, social work) OR a bachelor’s degree in another field with at least 3 years of related experience.

  • Minimum of 2 years of experience working with individuals with substance use disorders.

  • Community or public health experience is preferred.
