Salary: $75,000 to $78,000 (Depending on Experience)Purpose:
Responsible for participant care activities and other tasks as assigned to staff in the home or community care setting. Provides leadership to assigned staff. Keeps skills and certifications current through continuing educationEssential Duties:
1. Provide frontline leadership to assigned staff. This includes full employment decision responsibility and all aspects of supervision, including but not limited to recruiting, selection, on-boarding, training, coaching, developing, performance management, disciplinary action, and employment decisions.
2. Fosters team building, professional relationships, and good communication among staff and other departments. Routinely communicates and implements changes in program goals, philosophy, policy, and procedure.
3. Maintain staffing rosters, schedules, and daily assignments for assigned staff.
4. Ensure competency checklist completion with new staff members.
5. Report participant condition changes to IDT and participate in the IDT process.
6. Functions as the home care liaison with other disciplines.
7. Provide ADL care to participants as needed.
8. Ensure the assigned team produces accurate and timely documentation, including services provided in the home or other facilities. Document treatments/procedures, observations, personal care, and participant activities on appropriate forms and in the medical record as needed.
9. Performs work in various locations, including but not limited to PACE North, participant homes, nursing homes, assisted living facilities, adult foster care homes, skilled nursing facilities, hospitals, or other locations. Safely drives to and from assigned work locations outside of the PACE Center following all motor vehicle requirements.
10. In conjunction with the Quality department, perform audits of key performance indicators (KPI) and other scorecard monitors to ensure key metrics are achieved and quality measures are met.
11. Ensure that participant's rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to a prompt response to complaints, are handled properly.
12. Demonstrates active participation in QI plan processes. Attends meetings and/or group activities.
13. Follows all established PACE North policies, rules, and regulations, and all appropriate State and federal requirements. Works within certification and scope of practice.
14. Completes training and continuing education in a timely manner.
15. Performs work on-site, participant homes, and other community facilities in the assigned service area.
16. Assist other PACE departments and functions as needed and approved.
