Position Summary:
We are currently seeking a compassionate and motivated full time Psychosocial Rehabilitation Counselor to lead group and individual interventions within our Psychosocial Rehabilitation Program. This program supports adults with serious mental illness in developing the emotional, cognitive, and social skills necessary for independent living and community integration.
Key Responsibilities
Responsibilities include, but are not limited to:
- Designing and facilitating a structured program curriculum focused on improving clients' emotional, social, vocational, and cognitive skills.
- Documenting daily client progress and interventions in the Electronic Health Record (Credible) within 24 hours or by the next business day.
- Conducting timely annual assessments, individualized treatment plans, and treatment plan reviews.
- Maintaining accurate attendance logs with client signatures.
- Ensuring that all medical and clinical documentation complies with agency, state, and funding requirements.
- Participating in multidisciplinary treatment team meetings to coordinate client care.
- Supporting and growing the Medicare group by conducting intake assessments and facilitating individual sessions.
- Providing crisis intervention and therapeutic support to clients as needed.
- Serving as a backup to other staff during absences.
- Completing mandatory training requirements and continuing education as scheduled.
- Performing other duties as assigned by the supervisor or Clinical Director in alignment with agency goals and compliance standards.
Required Qualifications:
- Master’s degree in social work (LSCW), Clinical Mental Health Counseling (CMHC), or a closely related human services field from an accredited college or university.
- Minimum of two (2) years of experience working in mental health or human services.
- Experience with electronic health records (EHR), preferably Credible.
- Strong interpersonal, organizational, and written communication skills.
- Demonstrated ability to work effectively with diverse populations and interdisciplinary teams.
