Job Description
Schedule:
The schedule is Monday thru Friday from 8:00 A.M. to 2:30 P.M.
Social Worker (“SW”) Definition:
Pursuant the California Health and Safety Code – HSC §1570.7 (p), “Social worker” shall be a person who meets one of the following:
(1) The person holds a master’s degree in social work from an accredited school of social work.
(2) The person holds a master’s degree in psychology, gerontology, or counseling from an accredited school and has one year of experience providing social services in one or more of the fields of aging, health, or long-term care services.
(3) The person is licensed by the California Board of Behavioral Sciences.
(4) The person holds a bachelor’s degree in social work from an accredited school with two years of experience providing social services in one or more of the fields of aging, health, or long-term care services.
Social Worker Services:
(1) Interview and screen all referrals to determine the general appropriateness of the prospective participant for the full assessment process and adult day health care participation. T22 §54329 (a)(1).
(2) Provides in-home assessments for ADHC participants prior to admission.
(3) Provide an Initial Assessment, Quarterly Assessment & Reauthorization Assessment. Establish an Individualized Plan of Care (“IPC”). The IPC shall be based on the SW Assessment and should include a Problem, an Intervention, and a Goal. Act as a member of the Multidisciplinary Team.
(4) Promote peer group relationship through problem-centered discussion group. T22 §54329 (a)(6).
(5) Provide counseling to participants and their families on an individual and or group level.
(6) Serve as a liaison with the participant’s family and home. T22 §54329 (a)(7).
(7) Provide a signed and dated documentation for all service performed the day the services are provided and included signed and dated progress reports in each participant record. T22 §54329 (a)(3).
(8) Serve as liaison with other community agencies who may be providing services to a participant and work with these agencies to coordinate all services delivered to the participant to meet the participant’s needs. T22 §54329 (a)(8).
(9) Provide discharge planning for all discharged participants.
(10) All other duties as assigned.
Preferred Skills:
(1) Community-Based Adult Services (“CBAS”) experience.
(2) Bilingual
(3) Experience using of electronic-based programs for data management, record keeping and communications.
(4) Experience using MS Office, Adobe Reader, eFax and cloud storage.
Conditions for Employment
Must have current CPR + AED + Basic First Aid certifications, T.B. clearance and health exam within the last 12 months. Health exam must Include physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have a full range of motion; ability to perform up to a medium level of physical work. Medium Work involves exerting 20 to 50 pounds of force occasionally or 10 to 25 pounds of force frequently or an amount greater than negligible and up to 10 pounds constantly to move objects. Must have reliable record keeping skills. Must have reliable transportation. Must have proof of COVID19 Vaccination.
