Position Summary:
As a Social Worker, you will play a vital role in addressing the emotional, social, and psychological needs of our residents. You will work closely with an interdisciplinary team to ensure residents' well-being and support them through transitions in care, life events, and family dynamics.

Key Responsibilities:

Participate in and help coordinate interdisciplinary care plan meetings.
Conduct and document psychosocial assessments and progress notes.
Provide support and counseling to residents and families during admission, adjustment, and throughout their stay.
Assist with discharge planning, PASRR evaluations, and service coordination.
Serve as a resident advocate and connect families with appropriate community resources.
Ensure timely and accurate documentation in compliance with regulatory standards.
Maintain confidentiality and uphold resident rights at all times.

Qualifications:

Licensed in Massachusetts as an LSW, LCSW, LICSW, or LSWA.
Bachelor’s degree in Social Work from an accredited program required; Master’s degree preferred.
Nursing home or long-term care experience strongly preferred.
Familiarity with MDS 3.0 assessments, care planning processes, and PASRR evaluations is highly desirable.
Excellent interpersonal skills and the ability to work effectively in a team environment.
Empathy, patience, and strong problem-solving abilities.
