Position Summary

We are seeking a Care Manager to oversee the staff that support the residents and their needs. The ideal candidate will have experience in managing residential group homes and managing staff.

Key Responsibilities

  • Creating and updating personalized care plans for residents.
  • Collaborating with residents' families, friends and social support in developing treatment plans.
  • Identifying and recruiting high-quality care providers.
  • Interacting with healthcare providers to facilitate residents' treatments.
  • Ensuring residents’ treatment requirements are met.
  • Suggesting alternative treatment plans when residents' services requests do not meet medical necessity criteria.
  • Instructing and educating residents on procedures, healthcare provider instructions and referrals.
  • Linking residents to social services programs and entitlements such as transportation assistance and translation services.
  • Conducting regular follow-ups with residents to evaluate progress, promote continuity of care and ensure improved health outcomes.
  • Maintaining records of case management activities.
  • Create and manage weekly staffing schedule.

Education, Experience, and Licensing Requirements:

  • Bachelor’s degree in social sciences, social work, nursing or a related field.
  • 2+ years work experience in public health or related field.
  • Prior experience with supervision of staff required.
  • 1+ years current case management experience.
  • Certified Case Manager (CCM) license preferred.
  • Proficient with Healthcare Management Systems.
  • Outstanding communication skills, both written and verbal.
  • Excellent organizational skills.
  • Ability to travel locally.

Job Type: Full-time

Pay: $60,000.00 - $70,000.00 per year
