We are currently hiring multiple part-time MSWs to serve patients in their homes across Wayne, Macomb, and Oakland Counties. You’ll have the flexibility to choose your coverage area based on where you live or prefer to work.

Where You’ll Work

We’re actively hiring in the following areas:

• Wayne County: Detroit, Livonia, Dearborn, Westland, Canton, Romulus

• Macomb County: Warren, Sterling Heights, Clinton Township, Roseville, Shelby Township

• Oakland County: Southfield, Troy, Farmington Hills, Pontiac, Novi, Rochester Hills

Your Responsibilities

As a Medical Social Worker with Safe Hands, you will:

• Provide psychosocial assessments to identify patient and family needs

• Offer counseling and emotional support to patients and caregivers

• Coordinate community resources, financial support, and long-term planning

• Help families navigate end-of-life concerns and advance care planning

• Collaborate with interdisciplinary care teams including nurses, physicians, and therapists

• Document visits and care plans accurately using EMR systems

• Travel to patients' homes within your assigned county or cities

What We’re Looking For

• Active LMSW or LLMSW license in the state of Michigan

• At least 1 year of clinical social work experience, preferably in healthcare, hospice, or home health

• Strong interpersonal and communication skills

• Compassionate and culturally sensitive approach to care

• Familiarity with EMR and documentation standards

• Valid driver’s license and reliable transportation
