About Job

As Case Manager provides evaluation, consultation and intervention with youth and families. Also participates in conducting psychosocial programming with this population as part of a clinical team composed of Juvenile Probation, Mental Health staff, school personnel and other community providers in an effort to meet therapeutic goals and reduce the recidivism of high-risk youth within the Juvenile Justice System.
EXAMPLES OF WORK PERFORMED:

  • Works with youth and their families in mental health, home or office settings.
  • Provides case management and some skills training with children and families.
  • Makes assessment and psychosocial evaluations and recommendations.
  • Refers families of clients to appropriate social welfare and health agencies to obtain assistance.
  • Performs Related Work as Required.

Experience and Training:
One year of case management experience Preferred.
Must have current valid driver's license and current and continued proof of liability insurance.
Bilingual Preferred
Education:
Bachelor's/Master's Degree in Psychology, Social Work or related field.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Knowledge of emotional problems and treatment approaches.
  • Ability to establish and maintain positive working relationships with clients and colleagues.
  • Good work habits and organizational skills.
  • Familiarity with computer operation for clinical documentation.
  • Must have level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities as well as lift up to 30lbs.
  • Physical ability to perform CPR and PMAB skills.
  • Must have and maintain a level of general health which permits the performance of job duties.
  • Must have great written/verbal communication skills.
  • Travel locally 50% Regionally 5% and transport clients.
  • Ability to reduce recidivism.

Hours: Mon-Fri 8am-5pm with possible variations
Salary: $45,000-$52,965 DOQ
