Encore Village of Schaumburg is currently hiring for a full time Medicare Nurse Unit Manager/Clinical Care Coordinator. The Medicare Unit Manager will work in accordance with Briarwood Encore Village's established policies and procedures and/or specific instructions; and under the supervision of the Director of Nursing. The Medicare Unit Manager will have 24/7 reporting responsibility for clinical care coordination for the residents on the Medicare/Short Term Rehab Elm Unit and is responsible for overseeing the hands on resident care, proper and timely administration of medication and treatments, accurate verbal and written reports and rapid response to any/all emergency situations. Will be responsible for demonstrating, understanding, patience, tact, and courtesy in dealing with employees, residents, families and other visitors. Additionally, they will have the ability to supervise staff and have a good working relationship with them.
Responsibilities:
- Responsible for the implementation of Service Lines, such as Pulmonary Rehab and Cardiac Care to enhance quality of care, reduce hospital readmissions, increase physician and hospital referrals and ultimately admissions to Briarwood.
- Maintains knowledge of current long-term care regulations and programs in accordance with local, state, and federal guidelines.
- Ensures Medicare Certifications are completed and signed by the attending physician in accordance with Medicare guidelines.
- Responsible for ensuring the Unit is properly staffed according to resident acuity.
- Conducts rounds to monitor resident acuity and assess residents' physical and psychosocial status.
- Provides assistance with admissions, discharges and hands-on clinical care as needed to ensure a smooth flowing unit.
- Conducts Care Plan meetings with family and residents.
- Responsible for oversight of discharge planning protocols to ensure timely, safe and smooth resident discharges to the right level of living.
Certification/Education Requirements:
- Current RN Licensure with the State of Illinois
- Graduated from an accredited nursing program
- Current CPR Card
- Minimum 2 years experience working with geriatrics
- Clinical experience in coordination for residents in Medicare/Short Term Rehabilitation unit.
