The Silvercrest Center, proud member of the New York Presbyterian Regional Hospital Network, is a 320 bed skilled nursing located in Briarwood, Queens. About the Role

The Social Worker at The Silvercrest Center provides individualized and group social work services to residents and their families. This role is vital in supporting residents’ emotional and social well-being and ensuring their successful adjustment to facility life or return to the community. The Social Worker participates in interdisciplinary team conferences and collaborates with other departments to enhance patient care and outcomes.

Job Responsibilities:

  • Provide direct social work services to residents and families, including individual and group support, adjustment counseling, and discharge planning.
  • Carry an assigned caseload; complete timely documentation (e.g., MDS+, progress notes) and maintain accurate resident records.
  • Participate in interdisciplinary care plan meetings, providing social and emotional input into treatment planning.
  • Advocate for residents by exercising sound judgment, independent decision-making, empathy, and flexibility in addressing concerns.
  • Assist residents in accessing benefits and concrete services they are entitled to.
  • Support patient- and family-centered care: ensure communication is respectful, informative, and inclusive of resident/family participation.
  • Stay informed on community resources and programs; maintain updated resource files to assist residents and families.
  • Attend interdepartmental meetings and collaborate with other disciplines to address resident care and concerns.
  • Participate in and help implement Quality Improvement studies/projects.
  • Complete special projects and serve on committees or workgroups as assigned by the Director or Manager.
  • Supervise social work interns and volunteers, and assist in volunteer program operations including recruitment and recognition.
  • Assist in providing in-service training, especially around Residents’ Rights and related social work topics.
  • Clearly communicate facility policies and procedures to residents, families, and outside agencies.
  • Provide services tailored to the age-related physical and psychosocial needs of each resident.
  • Participate in continuing education to stay current with professional standards and facility requirements.
  • Demonstrate knowledge of and compliance with SCNR policies, including PPE and infection control protocols.
  • Perform additional duties as assigned in alignment with the mission and standards of The Silvercrest Center.

Qualifications

  • Bachelor’s or Master’s degree in Social Work (required)
  • Minimum of 1 year of related experience preferred
  • Experience working with geriatric and specialty populations is a plus
  • Strong communication, empathy, and problem-solving skills
  • Ability to work independently and make sound judgments
  • Knowledge of community resources and benefits
  • Willingness to participate in in-service training and ongoing education

Physical Requirements

  • Ability to lift up to 10 lbs and carry lightweight supplies
  • Frequent use of hands for typing, charting, and documentation
  • Must be able to remain in a stationary position for approximately 3 hours/day
  • May require occasional reaching above and below shoulder height
  • May be required to wear a surgical mask for extended periods
