Job Description: We are seeking a highly organized and empathetic Patient Intake & Enrollment Coordinator to join our team in Woodcliff Lake, NJ. This dual-role position is responsible for managing the intake process of new clients and coordinating the staffing of caregivers to meet client needs efficiently. The ideal candidate will possess excellent communication skills, a proactive attitude, and the ability to thrive in a fast-paced environment.
Responsibilities:
- Receive and process incoming referrals from hospitals, physicians, and case managers
- Collect and verify patient information, insurance details, and medical documentation
- Coordinate with clinical and administrative teams to ensure timely initiation of services
- Maintain accurate and up-to-date records in the agency’s EMR system
- Communicate professionally with clients, families, and healthcare providers
- Schedule initial assessments and follow up as needed to ensure service readiness
- Ensure compliance with all regulatory and agency-specific guidelines
- Respond promptly to changes in client needs and adjust staffing accordingly
- Collaborate with HR to ensure adequate staffing levels and assist in the onboarding of new caregivers
Qualifications:
- High school diploma or equivalent required; associate's or bachelor’s degree preferred
- Previous experience in a healthcare or home care intake and/or staffing role strongly preferred
- Excellent organizational and multitasking skills
- Strong interpersonal and communication skills
- Proficient in Microsoft Office and electronic medical records (EMR) systems
- Ability to handle sensitive and confidential information with discretion
- Flexible and adaptable with strong problem-solving abilities
