About Job

Job Summary: The Care Coordinator for Wound Care is responsible for overseeing and managing the wound care program for patients, ensuring the highest standard of care is delivered. This role involves helping to coordinate care plans with clinical staff, managing patient transitions, and collaborating with a multidisciplinary team to provide comprehensive wound care management. The ideal candidate will have previous experience as a discharge coordinator or social worker, and experience in home health, nursing, skilled nursing facilities, senior living communities, and/or wound care is highly preferred.

Key Responsibilities:

1. Care Coordination:

○ Assist in the coordination and development of wound care plans for patients.

○ Coordinate with healthcare providers, family members, and any other involved party to ensure continuity of care.

2. Patient and Family Support:

○ Educate patients and their families on wound care management and prevention.

○ Provide support and resources to patients and their families throughout the care process.

○ Act as a liaison between patients, families, and healthcare providers to facilitate communication and address concerns.

3. Administrative:

○ Assist in patient intake and handle referrals

○ Handle administrative duties - sending out faxes, handling referrals, ensure orders are completed and sent to appropriate parties

○ Collaborate with wound care specialists and other healthcare professionals to optimize patient outcomes.

4. Transition of Care:

○ Coordinate discharge planning and follow-up care for patients transitioning from hospitals or skilled nursing facilities to home or other care settings.

○ Ensure smooth transitions by communicating care plans and necessary information to involved facilities or caregivers.

5. Collaboration and Communication:

○ Work closely with a multidisciplinary team, including nurses, physicians, social workers, and therapists, to provide holistic care.

○ Participate in team meetings, case conferences, and other collaborative activities to enhance patient care.

○ Maintain effective communication with all parties involved in patient care.

6. Quality Improvement:

○ Participate in quality improvement initiatives to enhance wound care services and patient outcomes.

○ Stay updated on best practices and advancements in wound care management.

○ Ensure compliance with all relevant regulations, policies, and procedures.

Qualifications:

● Social Work, Healthcare Administration, or a related field.

● Minimum of 6 months of experience in wound care, discharge planning, or a related field.

● Previous experience in home health, nursing, skilled nursing facilities, senior living communities, and/or wound care.

● Strong knowledge of wound care treatments, protocols, and best practices.

● Excellent communication, organizational, and interpersonal skills.

● Ability to work independently and as part of a multidisciplinary team.

● Proficiency in electronic health records (EHR) and other healthcare management systems.

Preferred Qualifications:

● Experience in case management or care coordination.

● Knowledge of Medicare, Medicaid, and other insurance regulations related to wound care.

Working Conditions:

● These job duties can be performed in a hybrid setting - office and remote work. Majority of time in the office greater than 80%

Job Type: Full-time

Pay: $20.00 - $30.00 per hour
