Day to Day:

The social worker plays a vital role in supporting patients and families by assessing their social, emotional, and financial needs. They collaborate with healthcare teams to develop and implement discharge plans, provide crisis intervention, and connect patients with community resources. Additional responsibilities include participating in care planning meetings, maintaining accurate documentation, and offering after-hours support on a rotating basis.

Must Haves:

• BSW or MSW
• Minimum of 1-year medical social work experience (preferred acute care)
• Licensed to practice in Alabama
• Discharge Planning/Case Management Experience

Plusses:

• Acute Care – Social Work Experience Highly Preferred

Job Type: Full-time

Pay: $25.00 - $35.00 per hour

Expected hours: 40 per week
