Master Social Worker (MSW)

Location: Torrance, CA 90503
Schedule: Day Shift | 5x8-Hour (8:00 AM – 4:30 PM) | Every other weekend + holiday coverage
Contract Length: 13 Weeks

Job Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will provide essential support to patients and their families, assisting them in navigating the complexities of healthcare systems. This role involves working in various settings, including hospitals and nursing homes, focusing on the psychosocial aspects of patient care. The Medical Social Worker will engage in individual and group counseling, crisis management, and case management to ensure holistic care for patients.

Responsibilities

Perform biopsychosocial assessments and intake/admission screenings
Participate in developing individualized treatment plans
Coordinate discharge planning, including home health and long-term care referrals
Provide education, advocacy, and support to patients and families
Work with multidisciplinary teams to ensure continuity of care
Handle 5150 holds, worker’s comp, prior authorizations, and appeals/denials
Support patients in navigating Medicaid, Medicare, Social Security, and DCF processes
Weekend duties include overseeing up to two units with a focus on discharge planning
Document care delivery, plans, goals, and patient response in Epic

Qualifications

Master’s Degree in Social Work (MSW) – Required
Minimum 2 years of experience in acute care hospital case management – Required
Travel experience – Required
Experience with Epic charting system – Required
Strong knowledge of substance abuse, homelessness, mental health crisis support, and POLST
Must be able to work with patients of all age groups including neonatal, pediatric, adult, and geriatric

Job Types: Full-time, Contract

Pay: $55.00 - $60.00 per hour
