Clinical Magnet, a division of ICON Medical Network, is looking for a Home Care RN Case Manager to join our partner's team in San Bernardino, CA. This is a DIRECT HIRE OPPORTUNITY.

Key Responsibilities:

• Serve as a liaison between the patient, family, healthcare providers, and various agencies, ensuring smooth communication and coordination of care
• Conduct initial assessments in the patient's home to determine a patient's needs and determine the appropriate level of care and resources needed
• Create and ensure implementation of appropriate plan of care for participant based on findings from initial assessment
• Continuously monitor the patient's condition, progress, and the effectiveness of the care plan, making adjustments as needed and communicating any changes to the appropriate parties.
• Reassess patient's needs as often as required to determine continuity of care and determine if any changes are required/needed
• Other duties as needed

Key Requirements:

• Active, unencumbered Registered Nurse license in state of practice
• Associates degree in Nursing (ADN) required, BSN preferred
• Current First Aid and BLS certifications are required prior to hire.
• Reliable personal transportation required, current state issued driver’s license, good driving record and auto insurance as required by law.
• 1+ years of working with the geriatric population
• Home health experience required
• Bi-lingual-Spanish, Russian, Punjabi or Hmong is a huge plus!

Compensation and Schedule:

• Hourly rate begins at $44.66 but can be significantly more based on years of experience and education
• Comprehensive benefits plan
• Monday - Friday, 8AM - 4:30PM
• NO on-call!
