Duties
- Coordinate and manage care services for individuals in need of support.
- Develop and maintain care plans based on needs and goals of residents.
- Assist residents with daily activities, including meal preparation, personal hygiene, and mobility support.
- Utilize behavior management techniques to promote positive behaviors and reduce challenging behaviors
- Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care for individuals.
- Provide support in memory care programs, ensuring residents engage in meaningful activities that promote cognitive function.
- Conduct regular assessments of resident needs and adjust care plans accordingly.
- Lift and assist residents as needed while maintaining their comfort and safety.
- Document all care-related activities accurately in accordance with facility policies.
- Maintain accurate documentation of care provided and progress made by individuals
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments
Qualifications
- Previous experience in a similar role, such as direct support professional or care coordinator, is preferred
- Knowledge of behavior management techniques and systems is highly desirable
- Familiarity with care plans and the ability to implement them effectively.
- Strong interpersonal skills with a compassionate approach to assisting residents.
- Familiarity with home care services, memory care and regulations is a plus.
- Ability to lift and assist residents safely while adhering to best practices in caregiving.
- Excellent communication and interpersonal skills are essential for effective collaboration with individuals, families, and other professionals
- Excellent organizational skills with attention to detail to manage multiple tasks efficiently.
