Duties

• Coordinate and manage care services for individuals in need of support.
• Develop and maintain care plans based on needs and goals of residents.
• Assist residents with daily activities, including meal preparation, personal hygiene, and mobility support.
• Utilize behavior management techniques to promote positive behaviors and reduce challenging behaviors
• Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care for individuals.
• Provide support in memory care programs, ensuring residents engage in meaningful activities that promote cognitive function.
• Conduct regular assessments of resident needs and adjust care plans accordingly.
• Lift and assist residents as needed while maintaining their comfort and safety.
• Document all care-related activities accurately in accordance with facility policies.
• Maintain accurate documentation of care provided and progress made by individuals
• Monitor and evaluate the effectiveness of care plans and make necessary adjustments

Qualifications

• Previous experience in a similar role, such as direct support professional or care coordinator, is preferred
• Knowledge of behavior management techniques and systems is highly desirable
• Familiarity with care plans and the ability to implement them effectively.
• Strong interpersonal skills with a compassionate approach to assisting residents.
• Familiarity with home care services, memory care and regulations is a plus.
• Ability to lift and assist residents safely while adhering to best practices in caregiving.
• Excellent communication and interpersonal skills are essential for effective collaboration with individuals, families, and other professionals
• Excellent organizational skills with attention to detail to manage multiple tasks efficiently.
• A commitment to providing high-quality care while fostering a supportive environment for residents.
