About Outpatient Clinics

Corewell Health operates over 300 outpatient clinics across Michigan, offering diverse primary care and specialty practices. This variety provides opportunities for staff to explore different interests or to specialize in a particular area.

Scope of Work

Utilizes clinical judgment to coordinate care management and behavioral health interventions. Develops strategies to ensure cost-effective, quality care for patients with complex needs. Identifies appropriate levels of care for individuals and populations, focusing on promoting optimal health. Assesses, plans, monitors, and provides therapeutic interventions. Collaborates with patients and families to understand diagnoses and support lifestyle and behavior changes. Partners with healthcare teams to address the mental and emotional impacts of illness. Responds to crises and facilitates access to appropriate care levels. Duties may vary by area of assignment.

Qualifications

• Master's Degree in Social Work required
• LIC-Master Social Worker (MSW) or LIC-Master Social Worker LTD in Michigan required upon hire

Employment Type: Full-time

Shift: Day, 8:00 a.m. - 5:00 p.m., Monday to Friday

Weekly Hours: 40
