ONLY NEED SOCIAL WORK DIRECTOR WITH NURSING HOME EXP !

Nursing and Rehab center is looking to hire a talented and hard-working Director of Social Work (DSW). The ideal candidate will be a professional, compassionate and outgoing individual with work experience in the social services field, as well as experienced in the skilled nursing/long-term care setting.

Requirements:

· MDS completion experience

· Care planning

· Long term care department of health experience

· Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in directing the overall operation of the facility's delivery of services.

· Support state regulations for admission and retention of residents.

· Assist and interacts with residents and families to access services.

· Participate in development and implementation of each resident's individual service plan.

· Communicate with other members of the management team to ensure coordination of services to residents.

· Document case management notes in residents' files as necessary.

· Oversee and coordinate the Individualized Service Plan (ISP) and the ALR personal data and resident evaluation form.

· May perform other duties as assigned.

Qualifications:

· Minimum, 2 years’ experience in a supervisory capacity in a long term care facility or skilled nursing care facility

· Three years of active patient/resident contact in health care which includes at least two years of social services supervisory experience in long term care.

· Experience working with MDS, discharge planning, electronic medical records
