OVERVIEW OF POSITION:
The Client Care Coordinator is responsible for the overall coordination of services for families while residing in shelter. Client Care Coordinators work closely with homeless families and identify the need of mental health, medical, substance abuse, and crisis intervention services; they ensure that clients are link to optimal care and follow-up with treatment progression and compliance.
Duties:
- Responsible for the completion of psychosocial assessments for all families according to DHS time frame.
- Collaborate with city agencies, clinics, healthcare providers and other community organizations to facilitate linkages between families and community services.
- Identifies substance abuse, domestic violence, child abuse, trauma, and mental health conditions and determines appropriate treatment.
- Ensure that families are receiving full support in shelter and, when necessary, refer families for counseling or other services.
- Identify families and/or individuals who are in need of a higher level of service including; victims of domestic violence, families with active ACS cases, individuals exhibiting signs of mental illness or substance abuse, and non-compliant or hostile and aggressive clients.
- Collaborate with DOE, medical and mental health providers, early Intervention programs, ACS, etc., to ensure that the needs of children in our care are addressed.
- Participate in Social Services and Children & Youth Services Team meetings to address the needs of adults and children receiving clinical services.
- Facilitate workshops and groups as needed.
- Provide crisis intervention to clients.
NECESSARY SKILLS:
- Self-starter with a high degree of professionalism.
- Excellent oral and written communication skills.
- Must be highly organized and detail oriented.
- Must be highly proficient in Microsoft Office Suite, email and internet.
- Familiarity with office equipment.
- Ability to keep sensitive information confidential.
QUALIFICATIONS:
- Master's Degree in Social Work.
- LMSW or licensure within three months of hire.
- Knowledge of child and adolescent mental health, and diagnostic classifications.
- Knowledge of policies and regulations relating to child welfare, safety, and well‐being.
- Exceptional organizational skills and the ability to handle multiple projects and responsibilities simultaneously while maintaining high quality work.
Job Type: Full-time
Pay: $60,000.00 - $70,000.00 per year
