Intake Department Bilingual/ Spanish
Position Overview: We are seeking a highly organized and compassionate Home Care Intake Coordinator to join our team. The Home Care Intake Coordinator will play a crucial role in the initial assessment and onboarding process of clients requiring home care services. The ideal candidate will have excellent communication skills, a strong attention to detail, and a deep understanding of the needs and challenges faced by individuals seeking home care assistance.
Responsibilities:
- Client Intake and Assessment:
- Respond to inquiries from prospective clients and their families regarding home care services.
- Conduct thorough assessments of clients' needs, preferences, and medical history to determine the appropriate level of care required.
- Collaborate with medical professionals, social workers, and other stakeholders to gather necessary information for accurate assessments.
- Coordination and Scheduling:
- Develop and maintain a comprehensive scheduling system to ensure efficient coordination of home care services.
- Match clients with suitable caregivers based on their specific needs, preferences, and availability.
- Coordinate and communicate schedules with clients, caregivers, and relevant team members.
- Documentation and Record Keeping:
- Maintain accurate and up-to-date client records, including assessment notes, care plans, and relevant contact information.
- Ensure compliance with all legal and regulatory requirements by maintaining proper documentation and confidentiality.
- Client-Focused Communication:
- Serve as the primary point of contact for clients and their families during the intake process.
- Provide clear and empathetic communication about the services offered, pricing, and expectations for home care.
- Collaboration:
- Collaborate closely with the care management team to ensure seamless transitions from the intake process to ongoing care.
- Communicate effectively with caregivers to relay client preferences, needs, and any specific instructions.
- Foster relationships with current contracts.
- Problem Solving:
- Address and resolve any issues, concerns, or conflicts that may arise during the intake process.
- Identify potential challenges or obstacles and work proactively to find solutions.
- Continuous Improvement:
- Contribute to the improvement of intake processes by providing feedback and suggestions based on client interactions and trends.
- Seek new Referrals from insurance companies and the communities.
- Bachelor's degree in Social Work, Healthcare Administration, or a related field (or equivalent experience).
- Proven experience in intake coordination, case management, or a similar role within the healthcare or home care industry.
- Strong interpersonal skills with the ability to communicate effectively and empathetically with clients and their families.
- Excellent organizational skills and attention to detail to manage complex schedules and documentation.
- Knowledge of medical terminology, home care regulations, and healthcare ethics.
- Proficient in using computer software and databases for record-keeping and scheduling.
- Ability to work independently, prioritize tasks, and manage time effectively.
- Compassionate and patient-centered approach to caregiving and service delivery.
Salary:$22.00-$25.00 an hour.
Job Type: Full-time
Expected hours: 40 per week
