Job Description:
Day to day, you would be responsible for assisting in discharge planning of patients on the behavioral health unit, a 10-bed inpatient unit providing acute behavioral health services for those in need. Our interdisciplinary treatment team demonstrates their knowledge, critical thinking, and judgment. Services provided by the social worker include; completing assessments, communication with patients, families, collateral sources and community partners as well as the interdisciplinary treatment team, and facilitate psychoeducation group programming. Assists in the Discharge Planning needs of patients on the behavioral health unit. Member of an interdisciplinary treatment team and may assist in completing assessments within the scope of role and licensure if applicable, gathering information from and providing updates to collateral sources, and facilitate psychoeducational group programming in a behavioral health setting.
