About the Role
The Care Coordinator for Referral & Intake Management supports the end-to-end referral lifecycle by processing inbound referrals, managing intake forms, verifying insurance eligibility, tracking referral outcomes, and ensuring smooth patient onboarding. This role is essential to maintaining an efficient, high-touch intake process for TeleMed2U’s specialty care services.
Key Responsibilities
- Care Coordinator I:
o Process and review inbound referrals and associated documentation
o Ensure all intake forms and medical records are complete and accurate
o Verify patient eligibility and authorization status with health plans
o Monitor and manage waitlists and schedule patients accordingly
o Track dropped or missed referrals (“leakage”) and support conversion
o Send and manage intake paperwork for patients
o Upload patient documents and clinical records into AdvancedMD
o Manage redirect letters and communication with Primary Care Providers (PCPs)
o Track and complete PCP fax information as needed
o Support provider schedule optimization through proactive coordination
o Complete Emergency Department (ED) outreach tasks for CalOptima
o Manage CalOptima-specific intake tracker and requirements
o Handle unique client referral workflows (e.g., VB, Revive)
o Dock Health management
o Manage Online Scheduling (OLS)
o Conduct chart preparation prior to patient appointments
o Other duties as assigned
- Care Coordinator II:
o All Care Coordinator I responsibilities
o Serve as a liaison for training new team members
o Assist with problem solving and resolving patient concerns
- Care Coordinator III:
o All Care Coordinator I & II responsibilities
o Leads and is accountable for new team member training and development for 30-, 60-, & 90-day expectations
o Support Clinical Operations Managers and Directors in various responsibilities
o Serves as a team lead to disseminate updates on a routine basis.
o Foster overall team productivity
o Set team goals for daily, weekly, & monthly objectives
Qualifications
- Skills
o Bilingual (English/Spanish) a plus
o Knowledge of medical terminology preferred
o High accuracy in data entry
o Organized and detail-oriented
o Ability to follow intake checklists and procedures
o Basic insurance knowledge (eligibility and authorizations)
o Comfortable managing documents and uploads
o Clear written communication
o Strong time management
- Experience
o Care Coordinator I:
§ Prior experience in healthcare referrals, intake coordination, or a similar administrative support role preferred
§ Previous experience utilizing an EMR/EHR a plus
§ Telemedicine experience a plus
o Care Coordinator II:
§ Care Coordinator I requirements
§ Minimum 12 months experience at TeleMed2U or equivalent
§ Proficient on all areas of the EMR/EHR and can navigate seamlessly and complete most if not all functions
§ Demonstrates cross functional aptitude across most areas of the division
o Care Coordinator III:
§ Care Coordinator I & II requirements
§ Minimum 24 months of experience at TeleMed2U or equivalent
§ Expert in all systems, tools, and EMR/EHR functionality, as applicable
- Education
o High school diploma or GED required
Physical Requirements
- Prolonged periods of desk work and computer usage.
- Ability to lift up to 15 pounds.
