Wage Range: $85,000-$90,000 annually, depending on experience

Service Area: South Metro (coverage areas: Minneapolis, Edina, Richfield, Bloomington, Eden Prairie, Burnsville, Apple Valley, Chanhassen, Eagan, Inver Grove Heights, and surrounding areas). Plus, mileage reimbursement of .70 cents per mile.

Schedule: 1.0 FTE, Monday – Friday 8:00am-5:00pm (on-call every 4th week triage, Sun-Sat & rotation every 4th or 5th weekend) - On the weekends you are scheduled to work, you could be assigned visits covering all services areas.

The RN Case Manager is responsible to plan, organize and direct the Lifespark services while ensuring compliance with regulatory requirements and Lifespark best practices including but not limited to starts of care, care plan development and ongoing support, client satisfaction, and caregiver supervision, engagement and on-boarding.


Position Functions & Responsibilities:

  • Complete initial start of care documentation and process. Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates client nursing needs.
  • Initiates the plan of care and makes necessary revisions as client status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act.
  • Counsels the client and family in meeting nursing and related needs.
  • Provides health care instructions to the client as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
  • Acts as Case Manager and assumes responsibility to coordinate client care for assigned caseload.
  • Timely completion of all documentation as it relates to the care plan.
  • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse.
  • Completes orientation and annual training requirements as assigned.
  • Participates in Lifespark’s Event Management process including initiating reports, escalation of events and investigations.
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training.
  • Schedules and keeps member appointments.
  • Organizes travel efficiently and submits accurate auto use logs by established deadlines.
  • Tracks all billable activities.
  • Participates in on-call program rotation.
  • Participates in weekend coverage as needed.
  • Identify and communicate opportunities where there may be needs for Population Health, Life Managers, and other Lifespark services.
  • Perform additional responsibilities as assigned.

Qualifications:

  • Registered Nurse, BSN preferred
  • Prefer advanced certification as a Care Manager, Holistic Nursing, Geriatric CM, and/or Gerontology
  • 2 years’ RN experience in community-based services (assisted living, home care, hospice, public health)
  • Experience working with seniors preferred
  • Experience in customer service/direct member contact field preferred
  • Experience supervising caregivers
  • Must pass a MVR check based on Lifespark company policy
