Fort HealthCare is a patient-centered hospital and healthcare system serving individuals, businesses and families in Jefferson, Dane, and Walworth Counties.We are seeking a RN Patient Care Coordinator to join our Quality and Integrated Care team.

Shift:
Full time
Monday-Friday and every 5th Saturday
8-4:30

The RN-Patient Care Coordinators, together with the patient/family/significant other, clinical staff and physician will manage the care of the patient, including children, adolescents, and adults (including geriatrics), by collaborating with all members of the patient's care team.

Responsibilities of this position include:

  • Responsible and accountable for coordinating the care of patients across the continuum of care; ensuring and facilitating the achievement of quality, clinical and cost outcomes, negotiating, procuring, and coordinating services and resources needed by the patient/family/significant other.
  • Utilizes the nursing process to coordinate direct/indirect care of the patient meeting case management criteria through collaboration with team members within and outside of the organization.
  • Involves patient/family/significant other in identifying their needs and expectations to develop mutual and realistic goals.
  • Initiates and follows through with appropriate discharge planning in collaboration with the health care team and community resources.
  • Promotes wellness and illness prevention concepts and strategies for the patient/family/significant other.
  • Facilitates communication links, negotiates care provided with members of the health-care team, patient/family/significant other to ensure an appropriate treatment plan with timely and cost effective patient outcomes.
  • Coordinates and facilitates efficient use of resources by controlling duplication and/or fragmentation of care. Utilizes knowledge related to DRG's and reimbursement procedures in developing plan of care.
  • Performs admission, concurrent and discharge reviews for adherence to MCG criteria to ensure appropriate utilization of services.

Requirements of this position include:

  • ADN required
  • BSN preferred
  • 2-3 years minimum in area of practice required
  • Excellent understanding of the Nursing Process with emphasis on developing and maintaining care plans. Excellent communication and organizational skills. Diplomacy: ability to work well with all disciplines and levels. Acts as an agent for change. Ability to collect, interpret and act on data.
  • RN license to practice nursing in Wisconsin upon hire; BLS certification within department guidelines required
