Position Summary:

The Case Manager/Discharge Planner Assistant supports the facility’s Case Manager in ensuring effective discharge planning and coordination of care services. This role assists with documentation, communication with interdisciplinary teams, family support, transportation planning, and discharge logistics to ensure quality transitions of care and optimal resident outcomes.

Essential Duties and Responsibilities:

· Assist with the coordination of resident discharges, including scheduling home health services, DME, transportation, and follow-up appointments.

· Prepare and distribute discharge paperwork in collaboration with nursing and therapy departments.

· Assist in gathering insurance authorizations and communicating with payers or managed care case managers as needed.

· Help monitor and track discharge readiness and barriers in collaboration with IDT

· Maintain documentation and tracking tools for discharges, readmissions, and hospitalizations.

· Ensure the completeness and accuracy of discharge documentation and packets.

· Follow up on referrals to ensure services are initiated promptly post-discharge.

· Participate in morning meetings, weekly discharge planning meetings, and other relevant facility discussions.

· Maintain confidentiality and adhere to HIPAA guidelines.

Qualifications:

Education & Experience:

· High school diploma or GED

· At least 1 year of experience in a healthcare, case management, or long-term care setting is preferred.

· Familiarity with SNF discharge planning and transitional care services is strongly preferred.

Pay: $18.00 - $20.00 per hour
