Responsibilities may include:
- Providing patient visits as scheduled.
- Providing short term crisis intervention and individual or family counseling as needed.
- Serving as a patient and family unit advocate.
Requirements for this position include:
- Masters degree in Social Work preferred, Bachelors in Social Work accepted.
- License in Social Work from State of Pennsylvania preferred.
- Experience in Medical Social Work is preferred.
- Experience within the Hospice field is preferred.
- Valid driver's license, car insurance and reliable transportation.
- Willingness to travel throughout assigned service area.
