About Job

Vernon Rehabilitation and HCC is seeking a per diem Social Worker to support our Social Services team. The chosen candidate would have flexible hours to assist in the admission, discharge, and social, psychological, spiritual well-being of our residents, both short-term and long-term. The social worker's role should include:

  1. Planning for pre-admission and discharge
  2. Providing psycho-social assessment and completing relevant parts of the minimum data set (particularly face sheets, sections A, AA, AB, AC, E, F, Q, and sometimes B). Utilizing the minimum data set to guide the care plan
  3. Being aware of the worth and uniqueness of each individual
  4. Treating each individual with respect
  5. Creating an atmosphere of growth for the individual
  6. Adopting a holistic perspective by recognizing the dynamic interplay of social, psychological, physical, and spiritual well-being
  7. Providing a physical environment that is supportive rather than challenging or crippling to the individual
  8. Fostering a positive self-image for residents through continued social contact, decision-making opportunities, and independence
  9. Participating as a member of an interdisciplinary team in resident care planning
  10. Counseling residents and their families; contacting and utilizing community resources on the resident's behalf, and serving as a link between the resident and these resource systems when necessary
  11. Advocating for and protecting residents' rights
  12. Ensuring the social and emotional needs of each resident are met
  13. Stimulating social contact and interaction
  14. Promoting the maximum level of independence for each resident
