About Job

Job Purpose

The Social Worker is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of the Health Plan’s membership to include resource management and assessing Social Determinants of Health. This position assists the inpatient case management team (IP Hospital, ARU, LTAC, and SNF) with difficult discharge planning and community resources.

In addition, the Social Worker is responsible for contributing to the growth and success of HealthTexas while upholding our Mission, Vision and Values.

Essential Job Duties & Responsibilities


  • Professional verbal and written communication skills, with the ability to clearly articulate thoughts and ideas.

Experience


  • Minimum of 3 years of social work experience to include managing medical and socioeconomic issues.
  • Minimum of 2 years of public health or managed care Social Work experience preferred.
  • Knowledge of social support systems and resources is required.
  • Knowledge of applicable state, federal and third party regulations and standards.
  • Knowledge of local community resources available to the Medicare and Medicaid populations.
  • Personal computer experience including Microsoft Word, Excel, PowerPoint and Outlook at the intermediate level at a minimum.
  • Working knowledge of documentation in an EMR, preferably in ECW


Education

  • Bachelor’s degree in Social Work required: Master’s degree preferred.


Licensure

·        Required  Licensed Baccalaureate Social Worker (LBSW), Licensed Master Social Worker (LMSW), LMSW – Advanced Practice (LMSW-AP), or Licensed Clinical Social Worker (LCSW)


Knowledge, Skills & Abilities


·     Organization skills, able to handle multiple activities with changing priorities.

·     Strong written and verbal communication skills.

·     Competent in computer skills including MS Word, Excel, and Access.

                                              

Work Hours, Travel Requirements

·        Monday – Friday, 8:00 a.m. – 5:00 p.m., and as needed to complete projects.

·        Travel to local hospitals and/or rehab centers as needed to assist with placement, Medicaid applications, assistance with community resources, care planning meeting, and other assigned duties as needed.





Working Conditions & Physical Requirements

Requires prolonged sitting, some bending, stooping, stretching, and occasional lifting up to 50 pounds. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, and other office equipment. Requires normal range of hearing and eyesight to record, prepare and communicate appropriate reports. Standard medical office environment.
