Job Summary

Key Responsibilities
The Social Worker provides short-term therapeutic counseling and crisis intervention to patients and families, addressing psycho-social, financial, and home care needs. They collaborate with the healthcare team, assess patient needs, develop treatment plans, and ensure continuity of care. This role includes working with community resources, offering counseling, and resolving barriers to care.
Key Responsibilities
- Conduct assessments and create biopsychosocial evaluations.
- Develop and coordinate care plans in collaboration with the healthcare team.
- Provide therapeutic counseling and crisis intervention.
- Advocate for patients, connecting them with financial, community, and social services.
- Assist with discharge planning and continuity of care.
- Document interventions and outcomes.
- Participate in community outreach and education.
- Master's degree in Social Work (MSW).
- 2-3 years of healthcare social work experience.
- Licensure or eligibility for licensure in Indiana as an LSW or LCSW.
- Crisis intervention, counseling, and clinical assessment skills.
- Knowledge of community resources and healthcare social work functions.
- Strong interpersonal, communication, and problem-solving abilities.
