The Social Worker is a key member of the interdisciplinary team, responsible for assessing psychosocial symptoms of patients and families, developing outcomes, and providing psychoeducation, supportive counseling, and casework interventions. Key duties include:
- Conducting bio-psycho-social-spiritual assessments.
- Offering individual and family counseling.
- Referring to community resources.
- Maintaining accurate patient documentation.
- Participating in interdisciplinary care planning and team meetings.
- Providing bereavement care and psychosocial education.
