General Summary:
Provides clinical social work services and interventions to patient populations across the lifespan and continuum of illness, with a primary focus on addressing substance use disorders and related co-occurring behavioral health disorders. Performs psychosocial assessment and screenings, counseling, treatment and aftercare planning for patients in order to enhance patient motivation and willingness to change. Serves as a representative for Social Services, on committees, groups, and during community activities. Assists to prevent patient re-admissions. Provides social work services and discharge planning to patient populations across the continuum of illness, promoting effective utilization and a seamless transition to post-acute care. Performs crisis intervention with patients and families. Provides social work related support to the homeless and uninsured.
Responsibilities:
1. Works with the patient and patient's family and other interdisciplinary team members to develop a comprehensive assessment and understanding of the impact of substance use on the patient's overall physical and mental health. Develops a treatment and aftercare plan with the patient that includes the patient's strengths, needs, abilities, and preferences for treatment.
2. Utilizing an SBIRT treatment model, performs screening, brief interventions (motivational interviewing, solution-focused, and/or cognitive behavioral therapy techniques/interventions) and referrals to appropriate treatment resources.
3. Maintains pertinent and timely documentation on social work interventions, including statistical data. Completes identified and required medical and clinical assessment tools.
4. Provides education and resources to patients, families, and the community regarding substance use and co-occurring mental health disorders.
5. Serves as liaison and resource for members of the health care team.
6. Provide advocacy and resource services for the patient with a goal of enhancing a patient's motivation to change.
7. Develops and facilitates related support and self-help groups for patients and families.
8. Develops partnerships and relationships with key-community partners to help assist patients with a referral to the appropriate service and level of care.
9. Provides social work intervention and discharge planning services to patients and families in need of post-acute care.
10. This position may require occasional weekends and some evenings.
11. All other duties as assigned within the scope and range of job responsibilities
Required Education, Credential(s) and Experience:
  • Education: Master Degree
    ; Related field
    ;
  • Credential(s): Licensed Clinical Social Worker
    ;
  • Experience: Required: Two years' experience in a hospital or behavioral health social work setting. Preferred: Experience in working with patients with substance use/co-occurring disorders and complex medical needs.
