Job Summary
West Covina ADHC Center is seeking a compassionate and dedicated Social Worker to join our team at our Adult Day Health Care Center (ADHC/CBAS). This is a fantastic opportunity to work in a supportive environment that provides essential care and services to older adults and individuals with chronic medical, cognitive, or mental health conditions.

Duties

Conduct assessments to determine clients' needs and develop individualized service plans.
Provide counseling and support in areas such as addiction recovery, behavioral health, and childcare.
Utilize motivational interviewing techniques to empower clients in their personal development.
Collaborate with community organizations and agencies to connect clients with necessary resources.
Supervise and mentor interns or junior staff members in social work practices.
Maintain accurate documentation of client interactions, progress, and outcomes in compliance with regulatory standards.
Advocate for child welfare and participate in multidisciplinary teams focused on improving client services.
Stay informed about Medicare regulations and other relevant policies affecting client care.

Experience

A degree in Social Work or a related field is required; licensure is preferred.
Experience in workforce development, addiction counseling, or behavioral health is highly desirable.
Proven ability to work effectively with diverse populations and demonstrate cultural competence.
Strong interpersonal skills with the ability to build rapport with clients quickly.
Experience supervising or mentoring others is a plus, contributing to team development and effectiveness.
