We are currently hiring a Full-Time RN Hospice Clinical Care Manager

The Hospice RN Clinical Care Coordinator is a registered professional nurse who ensures quality of patient care in the home. Primary function is for the overall administration of the clinical departments and monitoring appropriate staffing and productivity. This role coordinates care with the Interdisciplinary Group/Team, the Patient and their family, and the referring agency. Further, this role monitors and analyzes QAPI issues and directs clinical staff with regard to the Hospice policies and procedures.

Responsibilities include but not limited to:

· Directs and coordinates clinical departments; assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations. (Conditions of Participation).

· Promotes the philosophy and administrative policies of the Hospice, in an effort to ensure quality of patient care.

· Participates in activities relevant to professional services including development of qualifications and assignment of the Hospice personnel.

· Supervises and provides direction to subordinates, in an effort to ensure quality, compliance with the Plan of Care, assessment and reassessment of patients’ needs, and continuity of services by appropriate health care personnel.

· Monitors the QAPI Program and assures appropriate corrective measures are performed.

· Promotes compliance with all fiscal intermediary and/or other third- party payors, through education, coaching, and other assistance as necessary.

· Participates as a member of the Interdisciplinary Group/Team (IDG/T), assists in development of the IDG/T plan, and participates in QAPI activities.

Qualifications

  • Graduate of an accredited school of nursing.
  • Licensed by the Oklahoma and/or Texas State Board of Nurses.
  • Minimum two (2) years experience as a registered nurse in clinical practice.
  • At least two (2) year of hospice experience.
  • Prefer candidates with one year or more of managerial experience.

Job Type: Full-time

Pay: $75,000.00 - $90,000.00 per year
