Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,5000 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting. Currently, we are seeking full time Care Manager to work in our Developmental Services Division.
- The purpose of this position is serve as a key liaison with external providers and service entities (e.g., Support Coordinators, job coaches, hospitals, CRCs, MH and D&A providers, law enforcement and judicial personnel, and others).
- To achieve and to maintain compliance with regulations within PA Chapters §6400 that reference the work/service responsibilities of a Care Manager
- To provide ongoing evaluation to monitor and ensure the necessary level of services required to promote participants’ individual's growth and recovery.
- To actively contribute to participants’ multidisciplinary and ISP meetings and planning processes
- To provide crisis intervention support and leadership.
- This position requires a Masters Degree in Human Services/Social Sciences/Healthcare or a related field and at least 2 years of experience in the field(s) of intellectual disability and/or behavioral health; or a Bachelors Degree and at least 3 years of experience in the intellectual disability and/or behavioral health field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
- Good analytical skills, organizational and planning skills, and therapeutic skills.
- Very good communication skills, including those pertinent to effective clinically-related documentation and writing service goals and outcomes.
- Ability to work independently and to contribute to team effectiveness.
- Frequent talking and listening.
- Possession of a valid driver’s license with an acceptable driving record.
- Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients.
Must be available for local travel.
