Job Overview
We are seeking a dedicated and compassionate Social Worker to join our Enso Hospice team. The ideal candidate will be responsible for providing support and assistance to individuals on hospice services. This role requires a strong understanding of patient care practices and therapeutic techniques to effectively assess and address the needs of clients.
Duties
- Conduct comprehensive patient assessments to evaluate individual needs and develop appropriate treatment plans.
- Provide psychotherapy and behavioral therapy sessions utilizing cognitive behavioral therapy techniques.
- Collaborate with healthcare professionals to coordinate care and ensure holistic support for clients.
- Maintain accurate documentation of patient interactions, treatment plans, and progress notes using medical terminology.
- Perform diagnostic evaluations to identify mental health conditions and recommend suitable interventions.
- Educate clients and families on available resources, coping strategies, and treatment options.
- Stay informed about ICD-9 and ICD-10 coding standards to ensure compliance in documentation practices.
- Advocate for clients' rights and access to necessary services within the community.
Requirements
- A Master's degree in Social Work (MSW) is required.
- Valid state licensure as a Social Worker.
- Experience in patient care settings with a focus on hospice care is highly desirable.
- Proficiency in cognitive behavioral therapy, behavioral therapy, and other therapeutic modalities.
- Strong understanding of medical terminology relevant to social work practice.
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
- Ability to work independently as well as part of a multidisciplinary team.
- Familiarity with diagnostic evaluation processes and coding practices is an advantage.
