Overview:
Provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation.
Responsibilities:
- Provides support to pediatric patients and families, in an outpatient clinic setting as part of a multidisciplinary team, to help address psychosocial needs within the context of the patient's medical care.
- Completes psychosocial assessments of pediatric patients and families to assess current supports, strengths, areas of need, and barriers to accessing care.
- Coordinates and connects patients and their caregivers with community resources to address/meet identified needs. Serves as a link between the medical team and the patient's community/natural supports.
- Assists caregivers in adapting to difficult and complicated dispositions involving complex care needs, multiple agencies/providers and linkage to resources in the community.
- Provides consultation, assessment, referral and coordination in cases of suspected child abuse or neglect.
- Organizes support groups, provides psychoeducational materials and opportunities for patients and their caregivers, based on their individual medical needs.
- Documents objective, concise and pertinent clinical information in the medical record.
- Perform other duties as assigned.
Qualifications:
- Master's Degree in Social Work (MSW) required.
- Social work experience in medical, school, or developmental services setting preferred.
- Strong organizational skills, verbal and written communications skills.
- Sound decision making and judgment.
- Time management skills.
- Negotiation skills.
- Proven ability to deal with conflict resolution.
- Comfortable with basic computer skills.
