Job Category: Outpatient ClinicsJob Description:Primary Duties and ResponsibilitiesThe Outpatient Care Coordinator assists with scheduling tests and treatments, including the coordination of multiple appointments amongst several different departments, programs and specialties. The responsibilities of this position include insurance verification, assisting with the pre-authorization process, and acting as a liaison between the pre-authorization team, physician office and patient. The Care Coordinator provides updates to the patient and doctors on timing and status of the authorizations as delays or insurance denying a pre-authorization could have an impact on the patient's health and/or testing and treatment options. Once the pre-authorization is complete and the appointments scheduled, the Care Coordinator provides the patient with an itinerary when possible. Assists with referrals as requested. Links family/patient to resources as needed. Performs other duties as assigned.Hoag Health Center - Jamboree: In addition to the above the Outpatient Care Coordinator provides support through a face to face, chat, texting or video platform. Must be competent in the use of Consumer Health technologies and software, for example, fitness wearables and health software applications.HMG - Prime: The Outpatient Care Coordinator will also fulfill Medical Assistant duties to support efficient patient access and workflow. Key responsibilities include the following: preparing necessary chart elements, recording vital signs, rooming patients, assisting with medical procedures as needed, and scheduling patients which can include follow up office visits, imaging and lab work, preparation of appropriate chart elements. The Outpatient Care Coordinator will work collaboratively with staff to ensure all proper records are received and all appropriate testing is completed prior to appointments.Qualifications:Education and Experience Required:
  • High School diploma or equivalent with 2+ years of patient access or related healthcare experience
  • Must possess strong knowledge in hospital scheduling, registration and insurance verification processes
Preferred:
  • 2-3 years hospital or outpatient based patient care coordination experience
  • Knowledge and understanding of patients benefits, plan coverage and claim responsibility
  • Hoag Health Center - Jamboree: Hospitality industry, concierge type experience preferred.
  • HMG - Prime: 2-3 years of medical assistant back-office experience.
