Job Summary
We are seeking a dedicated and compassionate Care Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the necessary medical attention and support throughout their treatment journey. The Care Manager will work closely with patients, families, and healthcare providers to facilitate effective communication and optimize health outcomes.

Duties

• Conduct comprehensive assessments of patients' health needs, including vital signs and medical history.
• Develop and implement individualized care plans in collaboration with multidisciplinary teams.
• Coordinate case management activities for patients in various settings, including hospitals, clinics, and emergency rooms.
• Ensure compliance with HIPAA regulations while handling sensitive patient information.
• Provide education to patients and families regarding infectious disease care and management.
• Utilize medical terminology effectively to communicate with healthcare professionals and document patient interactions accurately.
• Perform diagnostic evaluations to determine the appropriate course of action for patient care.
• Advocate for patients’ needs within the healthcare system to ensure they receive timely and effective services.

Experience

• Previous experience in case management or a similar role within a hospital or clinical setting is preferred.
• Familiarity with vital signs monitoring and basic medical procedures is essential.
• Knowledge of infectious disease care practices is a plus.
• Strong understanding of HIPAA regulations and commitment to patient confidentiality.
• Excellent communication skills, both verbal and written, with the ability to convey complex information clearly.
• Proficient in using electronic health records (EHR) systems and other healthcare technology.

Job Type: Full-time

Pay: $26.00 - $28.00 per hour

Expected hours: 40 per week
