POSITION SUMMARY:
DC Connected Care Network (DC CCN) is a clinically integrated network made up of participating health centers to increase quality and decrease cost. Goals include improvement to patient engagement and reduction of barriers to receiving high quality, primary care.
This role will be responsible for overseeing all DC CCN functions related to patient outreach and care coordination, in support of achieving quality and affordability targets. They will manage a program, set of functions, or team members, focused on supporting participating health center patient access and improvements to patient disease state management. Focus will maximize the alignment between value-based care contracts and federally qualified health centers.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Team Leadership & Management
- Supervise, mentor, and evaluate, including hiring and training
- Conduct regular team meetings to review cases, workflows, and performance metrics
- Foster a collaborative environment with participating center clinical staff, including nurses, physicians, social workers, and other providers
- Acts as an interface with other departments to coordinate workflow processes and implementation plans
- Identify, select, structure, and prioritize process improvement projects, ultimately implementing changes to meet program requirements
- Care Coordination & Patient Advocacy
- Develop and implement care coordination policies and procedures to improve patient outcomes
- Ensure timely and appropriate patient assessments, discharge planning, and follow-up care
- Address complex patient cases, resolving barriers to care (e.g., insurance, transportation, social determinants of health)
- Carry member case load as required
- Strong understanding of cultural fluency
- Quality & Compliance
- Monitor compliance with federal/state regulations (e.g., CMS, HIPAA) and accreditation standards
- Track and analyze key performance indicators (KPIs), such as readmission rates and patient satisfaction
- Lead quality improvement initiatives to enhance care delivery efficiency
QUALIFICATIONS:
Required:
- Bachelor's degree in healthcare administration, finance, or related field, and
- with a focus on patient interaction and commitment to high level patient quality experience
- Strong relationship management skills across stakeholders in a matrixed environment
Preferred:
- Clinical training and background (RN or equivalent)
- Excellent analytical, problem-solving, and decision-making skills
- Demonstrated track record of driving patient experience and disease state management outcomes
- Strong leadership, presentation, and communication skills (written and verbal)
- Strong relationship management skills
ADDITIONAL INFORMATION:
- DCPCA operates on a hybrid schedule: three days in the DC office and two days in remote office.
- Employees must reside in the DC metropolitan area.

ADA SPECIFICATIONS:
This position is primarily sedentary in the Washington DC office with occasional overnight travel and may require some bending and lifting. DCPCA is a fast-paced, high-energy organization with an ambitious agenda and a staff that is highly motivated. This position may experience high work demands under tight timelines.
