Position Summary

Develops referral sources, obtains and reviews referrals, and coordinates admissions to skilled nursing facility.

Responsibilities

 Develops collaborative relationships with hospitals, physicians, community organizations, team members and other referralsources.

 Develops and implements a marketing plan that will lead to referrals and admissions.

 Analyzes referrals to determine if the patient meets admission criteria both medically and financially.

 Coordinates the patient’s discharge from the hospital, home, or other setting and facilitates admission to skilled nursing facility.

 Prioritizes potential admissions based on the patient’s medical needs, financial support and discharge potential.

 Meets with residents/families to explain the admission’s agreement and obtain signatures.

 Coordinates opportunities for community groups to meet at skilled nursing facility.

 Represents skilled nursing facility at community events.

 Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.

 Maintains compliance with all Orlando Health policies and procedures.

Other Related Functions

 Enters referral information into the electronic referral log.

 Verifies insurance coverage and obtains authorization.

 Drives personal car and provides a monthly mileage expense report for reimbursement.

Qualifications

Education/Training

Associate degree, Nursing Degree from an accredited college or university, or graduate of an approved LPN program

Licensure/Certification

None

Experience

One (1) year experience marketing in the health care field.
