JOB SUMMARY:

We are seeking a compassionate Hospice Social Worker to provide essential social work services to our patients and their families/caregivers. In this role, you will conduct initial emotional, spiritual, and psychosocial assessments, offer ongoing counseling and bereavement support, and engage in community education, outreach, and referrals. As a key member of the Hospice Interdisciplinary Team (IDT), you will play a vital role in enhancing the quality of life for our patients during their end-of-life journey.


QUALIFICATIONS:

Educational/Degree and Licensure Requirements:

  • Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited school, with at least one year of experience in a healthcare setting.
  • Bachelor's degree in Social Work (BSW) from a CSWE accredited school, with at least one year of experience in a healthcare setting, supervised by an MSW with one year of healthcare experience (required for BSW holders employed after December 2, 2008).
  • Bachelor's degree in Psychology, Sociology, or related field, with at least one year of social work experience in a healthcare setting, supervised by an MSW with one year of healthcare experience.

Note: Compliance with state-specific social work licensure requirements is mandatory.


Knowledge/Skills/Abilities:

  • Ability to work independently, make accurate judgments, and respond effectively to crisis situations.
  • Acceptance and adaptability to diverse social, racial, cultural, and religious backgrounds.
  • Excellent interpersonal skills and ability to collaborate effectively within interdisciplinary teams.

Experience: Minimum of 2 years of experience as a social worker, with active patient contact within the past three years preferred.


JOB FACTORS:


Physical Requirements:

  • Ability to perform physical tasks including kneeling, squatting, reaching, climbing, and minimal lifting/transferring of patients.
  • Adequate sight and hearing or use of adaptive devices as necessary.

Mental Requirements:

  • Ability to work under minimal supervision, make informed decisions based on assessments, and demonstrate emotional stability and flexibility.

Working Conditions:

  • Willingness to work in varied environments including exposure to odors, blood, body fluids, and hazardous materials.

Transportation:

  • Possession of a valid driver’s license, auto liability insurance, and reliable transportation.

Essential Functions:

  • Conduct initial assessments and assist in developing comprehensive care plans.
  • Provide ongoing counseling, crisis intervention, and bereavement support.
  • Actively participate in IDT conferences and advocate for patients and families.
  • Maintain detailed and timely documentation of patient interactions and care plans.
  • Foster relationships with community agencies to facilitate referrals and promote interagency cooperation.
  • Attend meetings and training sessions as required, adhering to all Hospice policies and exhibiting our hospice philosophy in all aspects of your work.

Other duties as assigned by Director/Manager of Patient Services.
