Job Title: Home Care Resident Care Coordinator
Work Status: Exempt, Full Time
Work Hours: Monday-Friday, Dayside Hours
Rate of Pay: $46,000-50,000 annually based on experience

Sedgebrook is looking for a hospitality-focused Home Care Resident Care Coordinator! This role is primarily responsible for the resident experience and coordination of care, which includes accurate and timely assessments, plan of care, and maintaining visit notes, tasks, and their charts.

Here are a few of the daily responsibilities of a Home Care Resident Care Coordinator:
- Manages the Resident experience and their care services.
- Acts as the primary contact for new and current clients, and their families/responsible parties.
- Responsible for all new client service agreements, onboarding, and paperwork; and maintaining those files.
- Conducts timely resident assessments/plan of care updates and agreements for home care services. Participates in discussions and meetings with residents and their families regarding services needed to remain safe in their apartment home.
- Ensures proper maintenance of client records in compliance with state laws.
- Creates and maintains the Resident's Plan of Care, visit notes, and files.
- Follows up with hospitalized residents and residents at Radford Green.
- Conducts caregiving check-ins and shadowing, as well as supervisory visits.
- Keeps supervisor informed of client status and any related issues.
- Participates in after hours on-call rotation.
- May need to perform personal care and assistance to the residents in their homes in an emergency.
Here are a few of the qualifications we need you to have:
- High school graduate or equivalent required.
- Associate's or Bachelor's degree preferred.
- Minimum one (1) year experience working with seniors providing dementia level care, and at least one (1) year in a home care agency setting.
- Knowledge of good safety practices in working with clients with physical limitations.
- Basic mathematical skills required.
- Exceptional organizational skills.
- Must have strong customer service skills and passion for working with older adults.
- Ability to work evenings and/or weekends, as needed.
- Ability to read, write and speak effectively in English.
- Computer literacy required; Proficiency in word processing and computer skills (Office, Excel, PowerPoint, and scheduling software).
- Current first aid and CPR certification or must obtain within 3 months of hire.
