Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

The Compact Act Care Coordinator (CACC) is responsible for the implementation, coordination and monitoring of the COMPACT Act at the facility.

Major Duties include but are not limited to:
  • Navigation between varying levels of care.
  • Coordination between health care providers and care managers in direct care and community care.
  • Comprehensive transition planning.
  • Outreach to community providers and healthcare systems providing COMPACT care.
  • Advocacy for Veterans, ensuring they do not receive bills for COMPACT related care.
  • A singular point of contact for Veterans, families and caregivers during COMPACT episodes of care.
  • Facility subject matter expert for COMPACT Act.
  • Coordinate with local Patient Advocate Tracking System - Replacement (PATS-R) COMPACT clinical review teams.
  • Collaborate with their local facility revenue and finance departments.
  • Coordinate with their local facility Office of Integrated Veteran Care (IVC) Coordinators.
  • Ensuring proper notes are used for Veterans receiving care under the COMPACT Act authority in order to ensure they are not billed for crisis care.

Work Schedule: Monday - Friday, 8:00am - 4:30pm

  • You must be a U.S. Citizen to apply for this job.

  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).


Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
