AGENCY DESCRIPTION:

Instituto Familiar de la Raza, Inc. is a multi-service community health and social service agency with an emphasis on serving the Chicano/Latino/Indígena community in San Francisco with a special focus on the diverse needs of the Mission District. Services include a wide range of mental health, HIV-related services, and social services, including health promotion, education, prevention, early intervention, case management, psychological and psychiatric interventions, and cultural/social and spiritual re-enforcement. The agency serves children, youth, adults, and families. It has a rich history of working collaboratively with other agencies to meet the needs of the Chicano/Latino/Indígena community and other cultural/racial communities in San Francisco.

PROGRAM/POSITION DESCRIPTION:

Promesa LGBTQ+ Youth Services is a program within IFR’s Si a la Vida initiative, providing holistic, culturally responsive, and affirming support to LGBTQ+ Latinx youth, with a focus on newcomer and monolingual Spanish-speaking youth (ages 14–24) in San Francisco.

Rooted in harm reduction and restorative justice, the program addresses barriers these youth face at the intersection of immigration, race/ethnicity, gender identity, and sexual orientation. Services include: motivational interviewing, individual care management, group support, outreach, community education, and youth-led storytelling/media projects.

Through a trauma-informed and client-centered care approach, Promesa fosters safety, resilience, and leadership, creating opportunities for LGBTQ+ Latinx youth to thrive in the community.

The Care Manager plays a critical role in supporting LGBTQ+ Latinx youth (ages 14–24) by providing strength-based, culturally appropriate care management and advocacy. This position also supports youth outreach, group facilitation, and storytelling/media initiatives and provides referrals as appropriate to address barriers related to immigration, language, mental health, and discrimination.

Essential Job Duties include but are not limited to the following:

  • Develop and implement care management activities that are youth-centered, community-informed, and grounded in cultural humility.
  • Support youth with developing their Individual Service plans and goals that are strength-based.
  • Provide ongoing care management services, including regular individual support, referrals to appropriate services and follow-ups.
  • Maintain timely and accurate records of participant contacts and service delivery.
  • Meet program goals and deliverables by maintaining an active careload.
  • Facilitate or co-lead weekly youth support groups and community workshops.
  • Collaborate with partner agencies, community organizations, and SFUSD schools to identify, engage, and recruit eligible youth participants into the program.
  • Build and maintain relationships with community-based providers, schools, and youth-serving agencies.
  • Participate in cross-program collaborations and community events.
  • Maintain consistent communication with the supervisor and team regarding the youth's progress and concerns.
  • Other duties as assigned.

REQUIRED SKILLS:

  • Bilingual and bicultural in English and Spanish (required).
  • At least 1–2 years’ experience working with LGBTQ+ youth and/or immigrant communities.
  • Knowledge of harm reduction, sex positivity, youth development, and restorative justice.
  • Experience in group facilitation, outreach, or leadership development for youth.
  • Proficient in Microsoft Office and comfortable using digital tools.
  • Ability to maintain confidentiality and professional boundaries.
  • Flexible schedule (some evenings/weekends required).
  • Strong communication and self-organization skills.
  • Ability to de-escalate conflict and support diverse populations.
  • Passionate about community wellness, social justice, and youth empowerment.

EDUCATION, CERTIFICATION AND LICENSES

  • Bachelor’s degree (or equivalent) in social services or health field and/or Community Health Worker certification and/or relevant certifications or training in care management practices (ex., Certified Care Manager (CCM) or Certified Social Work Care Manager (C-SWCM))
  • A combination of relevant work and education and training in the areas of social welfare, youth development and/or community advocacy can be considered.
  • Valid Driver’s License

CONDITIONS OF CONTINUED EMPLOYMENT

  • Must pass and maintain annual Tuberculosis Test
  • Must pass the Department of Justice fingerprint screening

Compensation And Benefits²

Salary Range: This is a full-time position with benefits, with an hourly salary range of $27.08 to $30.00, commensurate with work experience, qualifications, and education. All full-time positions are based on a minimum of thirty-five (35) hours per week.

This is a full-time non-exempt position at 35 hours a week. The position is eligible to participate in medical, dental, vision, and 403(b) plans, earn vacation, sick leave, paid holidays, two wellness days (birthday and hire date) and receive ongoing learning opportunities.
